MOLE REMOVAL THAT LEAVES NO SCAR! - Introducing the Plasma Fibroblast

DIAGNOSTIC IMAGING UNDER THE SKIN

10/21/2022- Ms. January Howard (Ft. Lauderdale, Fla), national expert trainer in minimally invasive facial enhancement procedures (Chemical Peels, PRP, Micro-needling, Biologic fillers) meets renowned medical diagnostic imaging specialist Dr. Robert L. TRAUMA AND TOXINS: Non invasive diagnostics

The human body is continually assaulted by harmful forces which may be obvious (trauma and burns) or subtle and dismissed as the “flu or nerves” (chronic poisoning and delayed hidden scarring). However, in the unregulated world of fillers, patients and physicians often discover unexpected findings (forgotten surgical sutures) and complication as potential medicolegal traps. One picture is worth a thousand words and one image may launch a thousand lawsuits while possibly giving birth to a new medical image guided treatment paradigm. 

TRAUMA
Soft tissue trauma causes a black and blue area but subcutaneous pathology is best imaged by ultrasound. The normal dermal layer is light gray on scans while inflammation is dark gray and fluid (blood) is black. Dermal ultrasound has been used for 30+ years to find skin cancer and guide scar treatment so the use in subacute trauma victims is a logical progression of this portable and non invasive technology. Foreign bodies such as glass and splinters are easily visible as bright white reflectors that directs the surgeon to the exact removal site under ultrasound guidance with minimal tissue “exploration” Fillers have characteristic echo pattern where HA products appear as black globules when they coalesce.  Often the HA injected aliquot disperses immediately leaving a diffuse hazy picture. Complications of fillers are well described in recent textbooks. A special case is free silicone having specific “snowstorm” pattern that is commonly seen in breast imaging of ruptured implants. The theoretical possibility of immune system compromise by free silicone is still being studied.

FIBROTIC SCARRING
Elastography shows scar tissue quantitatively in the liver parenchyma but also in traumatized muscles and tendons. The “elastic” properties of tissue are used worldwide for cancer diagnostics because malignant tumors are rock hard and “gritty” with the needle biopsy while benign lumps are soft. Ultrasound maps tissue signatures of free silicone has a mean gray (MG) value 35-40 on a black to white scale.

SKIN OF COLOR
The headline from the 2022 fall issue of PSORIASIS ADVANCES noted inflammatory disease is often misdiagnosed in skin of color. Bruises, burns and infections are detected by color-blind ultrasound as dark areas in the light gray tissues often highlighted by a “ring of fire” blood flow reaction to the local tissue reaction 




(Fig 1-L) 45 year female with collagen disease chief complaint of  fullness post filler. Sonogram at 18MHx shows a 0.2mm epidermis which expands to 9mm over the tender area. Black HA filler caps the 16x16mm light grey focus of free silicone. Under ultrasound guidance the needle depth was recalculated to avoid injecting and possibly dispersing the silicone material.

From Smart Research to the Consultation

Whether you're searching for a primary care physician or a cosmetic specialist, shopping for any doctor can be quite a challenge. Ingredients for finding a professional that you can trust with your life rely heaviest on their reputation, expertise and qualifications found under smart research methods.

REFERRALS: As medical professionals are now heavily participating in commercial marketing platforms (such as broadcast and podcast commercials, Youtube videos, social media promotion and public relations campaigns), the most favored influencer about any practitioner is still word of mouth. At least starting with a recommendation from a friend, colleague or family member, getting positive feedback about a practitioner through others' experience is a great place to begin.

RESEARCH: Once you have a name (or several names to choose from), the next answer is conducting online searches. With more than a decade of evolution, the internet provides a wide expanse of research options of about any professional. From patient reviews, local directories, educational contributions, social media connections and background checks, the web helps with so many ways to check out any professional for any success as well as not-so-good-news about them.

Meanwhile, as you proceed with your online research, employ 'smart searching' by knowing where to look and what to look for. Do not make quick judgments about an individual based on a sexy looking website. There are many doctors out there that have little marketing sense that pay more attention (as they should) in working with their patients than working with a fancy graphic design firm. On the other hand, doctors' websites may come with video testimonials, news coverage, educational backgrounds and a list of their partners- all information that help describe the strength of their capabilities. All doctors carry a varied set of strong points academically and professionally and it's up to you to find the clues you need to make that choice.


Narrowing down your candidates also means being organized. Have a folder and notes on significant information gathered online and/or from others. Save links that show vital information from reputable websites that you can return to for comparative reviewing. In the end, sizing up a doctor is all about the collected facts about them that you choose to dictate your decision.

E-REVIEWS: Affirming our freedom of speech is the internet's best function. Reviews, ratings and consumer reviews are aplenty - allowing you an inside look at a doctor's performance but discern with caution when finding either a "sugary" five-star comment or a demoralizing bad opinion in YELP (or the various popular review websites). Over time, you'll find many nay-sayers freely and maliciously bashing a professional for something completely unrealistic like "They had me sitting in the waiting room for 25 minutes without anything to read - so unprofessional!" In other words, discern the SOURCE of the review and conduct a fair amount of research instead of taking the first available opinion.

MEET AND GREET: Doctors come with their own personalities that may or may not match yours. During the first consultation, you may get the best clues as to whether or not this particular doctor is for you. Just as with any other relationship-based professional, follow your instincts during the inquiry of your ailments. You can usually tell if they're rushing to get 'done with you' or if they truly care about exploring your issues through genuine concern for problem-solving. Select a doctor who shows true interest in recommending current techniques, applications and solutions and one who is open to remedies outside of the pharmaceutical box. Know that the healthcare field is a complex field of information that is constantly growing and requires continued education to provide the most current care.

YOUR COMFORT LEVEL: Intuition is your inner voice telling you what's right for you. Your comfort level can be driven by a host of logical and illogical factors varying from the doctor's gender, their location, the design style of their office, their bedside manner or something as silly as their hairstyle. We may not always be able to identify what it is about a doctor that you are either drawn to or may have some apprehension about but at the end of the day, it's how you feel about that practitioner.

The ultimate answer to matching with the "right" doctor is truly based on a per-patient case. Where recommendations may fail, follow your gut, research as much as you can and remember, if you're in doubt or have concerns, NEVER SETTLE! There's always a second, third or fourth opinion and a wide list of other docs to choose from.

THE ARM LIFT

FACTS ABOUT LIPOSUCTION
The results of liposuction can be long lasting, provided you maintain a stable weight and general fitness.  Your improved body contour will be apparent when the swelling and fluid retention commonly experienced following liposuction subside. As your body ages, it is natural to lose some firmness, but most of your improvement should be relatively permanent. Although good results are expected from your procedure, there is no guarantee. In some situations, it may not be possible to achieve optimal results with a single surgical procedure and another surgery may be necessary. Mr. Bilirakis, Mr. Speaker. I rise today to commend the work of one of my constituents, Dr. Paul Dreschnack-- a plastic surgeon who spends several weeks each year in India, voluntarily performing free operations on children born with facial defects. I recently nominated Dr. Dreschnack and his mentor, Dr. Sharad Kumar Dicksheet, for a Nobel Peace Prize. I would like to share with our colleagues the letter I submitted with their nomination application.   On behalf of the United States House of Representatives, I thank Dr. Dreschnack and Dr. Dicksheet for their tireless work.  They are very worthy of this prestigious award and would uphold its tradition of outstanding recipients if it is awarded to them
Dear Nobel Committee Members: It is my distinct privilege to bring before the Committee two physicians whose humanitarian contributions in the area of medicine have prompted me to submit their names for consideration as Nobel Prize Laureates. I became acquainted with the work of Dr. Dicksheet and Dr. Dreschnack during a recent meeting with representatives from a local chapter of an international civic organization, the Rotary Club of Dunedin, North.  The story that unfolded over the next several hours could be submitted by the headlines of some of the articles contained in their packet: "The Doctor's Heart: A New York Doctor Returns to India to Give His Life's Earnings Back": "New Life to the Deformed"; "One Man, 20,000 Lives."
